Full-Time; Salary $39.90 to $43.37 per hour
Position Purpose
Reporting to the Program Manager, Integrated Service Delivery Planning and Partnerships, is responsible for coordinating the service planning and partnership building with the province, local municipalities and service agencies for the purposes of establishing multi-service locations and community hubs; planning and project managing future multi-service/community hub locations; exploring opportunities to partner with internal and external stakeholders; working closely with Strategies and Partnerships in gathering and analyzing data to identify community needs; and liaising with various internal and external partners for project implementation.
Major Responsibilities
  • Leads in project planning, implementation and evaluation of community hub/co-location projects.
  • Identifies funding opportunities for community hub projects and prepares proposals as necessary.
  • Works closely with local municipalities, community partners and the public to establish the vision for specific community hub/co-location projects. 
  • Analyzes program activities and identifies future needs including service gaps in the community.
  • Works closely with community based agencies to determine capacity development needs and establishes and implements community hub/co-location projects to meets those needs.
  • Develops and maintains an evaluation strategy to assess the effectiveness of the community hub initiatives through the collection of data, input and feedback from tenants and the community.
  • Researches community hub best practices to guide the development of future community hubs in York Region.
  • Identifies alignments with provincial direction on community initiatives.
  • Liaises with corporate partners to ensure successful delivery of hub projects.
  • Liaises with local municipalities, community agencies and other external partners with regard to joint partnership opportunities.
  • Successful completion of a University Degree in Public Adminstration, Public Policy, Social Sciences or approved equivalent combination of education and experience.
  • Minimum three (3) years experience in community engagement and development and/or project management related to the Human Services or Health sector including planning, analysis, program/project development and management, program evaluation, research, public consultation, and change management.
  • Experience working with community agencies, representatives from various levels of government and managing multi-stakeholder projects.
  • Experience facilitating workshops and consultation sessions.
  • Understanding of core concepts and models of service integration, community development, service delivery hubs and community leadership.
